Telcare, Inc. and Health Dialog Announce Strategic Partnership for Enhanced Diabetes Management

According to the Centers for Disease Control and Prevention (CDC), diabetes affects 25.8 million Americans. If trends continue, more than a third of the country will have the disease by 2050. In 2012, the estimated total cost of diagnosed diabetes in the United States was $245 billion.

To help patients, physicians, employers, and health plans better address this growing epidemic, Telcare, Inc., the leader in mobile diabetes management solutions, and Health Dialog, a preeminent provider of care management, today announced a strategic partnership. Under terms of the agreement, Telcare’s blood glucose monitor (BGM) will integrate with Health Dialog’s diabetes management program, providing patients living with diabetes and their healthcare providers a more effective way to manage and control the chronic condition.

Using glucose values transmitted in real-time by the Telcare BGM, patients now have instant access to feedback and coaching for their condition. Caregivers and providers have online access to the same results, allowing them to easily analyze the data and get alerts about potential patient complications. Additionally, patients no longer need to keep a log of their blood glucose readings to convey to their provider. Providers can spend more time during patient appointments focusing on treatment and less time reviewing blood glucose levels.
