Susquehanna Health Adds Online Health Portal to Internal and Employer-Directed Population-Health Programs

Susquehanna Health is making OneCommunity, an online population-health portal powered by Aegis Health Group, available to businesses across their service area in north central Pennsylvania. This interactive Web solution allows consumers to track their health and wellness activities and will be used by Susquehanna Health employees as well as the employees of local businesses engaged in the hospital’s employer-based wellness program.

“Improving the health of the local community is a top priority for Susquehanna Health,” said Chris Ray, the health system’s director of referral development. “OneCommunity allows us to interact with the program’s participants in an engaging way while providing an opportunity for users to monitor and track their health status.”

OneCommunity is an online population health portal that stores individuals’ personal health profile data and biometric results, such as cholesterol, blood pressure and blood glucose levels. Each user’s account is password-protected and compliant with the Health Insurance Portability and Accountability Act (HIPAA). The aggregated information available through OneCommunity helps employers anticipate and mitigate health risks using solutions offered by the hospital, resulting in a healthier and more productive workforce.
