Susquehanna Health Adds Online Health Portal to Internal and Employer-Directed Population-Health Programs

Susquehanna Health is making OneCommunity, an online population-health portal powered by Aegis Health Group, available to businesses across their service area in north central Pennsylvania. This interactive Web solution allows consumers to track their health and wellness activities and will be used by Susquehanna Health employees as well as the employees of local businesses engaged in the hospital’s employer-based wellness program.

“Improving the health of the local community is a top priority for Susquehanna Health,” said Chris Ray, the health system’s director of referral development. “OneCommunity allows us to interact with the program’s participants in an engaging way while providing an opportunity for users to monitor and track their health status.”

OneCommunity is an online population health portal that stores individuals’ personal health profile data and biometric results, such as cholesterol, blood pressure and blood glucose levels. Each user’s account is password-protected and compliant with the Health Insurance Portability and Accountability Act (HIPAA). The aggregated information available through OneCommunity helps employers anticipate and mitigate health risks using solutions offered by the hospital, resulting in a healthier and more productive workforce.

“OneCommunity is the perfect complement to Susquehanna Health’s population-health management program, both within their own workforce as well as with the employers they serve throughout the area,” said Yale Miller, Aegis Health Group’s executive vice president of operations. “Consumers appreciate the convenience of being able to securely access their personal health information anywhere and at any time available through an online population health portal.”

Aegis’ time-tested tools, currently used by leading health systems across the country, support facilities like Susquehanna Health as they seek to match at-risk individuals with appropriate programs and services offered by the hospital. Healthcare reform has underscored the value of worksite wellness programs through its emphasis on health improvement and disease prevention. The law also has increased the value of incentives employers can offer to encourage worker participation.

Digital Transformation is the ultimate goal of cloud computing and related initiatives. The phrase is certainly not a precise one, and as subject to hand-waving and distortion as any high-falutin' terminology in the world of information technology. Yet it is an excellent choice of words to describe what enterprise IT—and by extension, organizations in general—should be working to achieve. Digital Transformation means: handling all the data types being found and created in the organizat...
The Software Defined Data Center (SDDC), which enables organizations to seamlessly run in a hybrid cloud model (public + private cloud), is here to stay.
