By allowing care teams to prescribe actionable and interactive CarePlans, FitangoCare enables health care to extend beyond the hospital or doctor's office to the patient's home and beyond via web and mobile access. CarePlans are equipped with personalized, condition-specific information and adherence trackers that provide patients with valuable insight into their medical conditions and health habits. FitangoCare also empowers patients by providing 24/7 web and mobile access to the actionable information they need to improve their health without the confusion of traditional verbal and paper-based care instructions. This new process is revolutionizing the way patients receive and interact with their health care.

Fitango's CEO and founder, Dr. Dov Biran, said his goal was "to develop an innovative solution that would help patients better understand and take control of their health and treatments, while providing value and benefits to providers and payers."

Designed to support meaningful use and address health risks in post-acute and chronic care populations, FitangoCare allows care team members, such as discharge planners, nurses and care coordinators, among others, to proactively and efficiently monitor patients' adherence levels and vital signs in real time. By also connecting patients to their social support networks and care team, FitangoCare enhances adherence and communication, significantly improving outcomes.

"Fitango has completed beta testing and will continue refining FitangoCare throughout the launch and early market stage. We look forward to working with our early adopters to ensure that we provide a flawless and ever-evolving solution that will comply with future market requirements. We want to make certain that FitangoCare will effectively benefit providers, payers, and most importantly, the 130 million Americans affected by chronic diseases and behavior related conditions," Dr. Biran stated. "We are very excited to share our innovative solution with the market, as it can truly make a big difference in the American health care landscape."
