Buena Vida y Salud Selects Sandlot Solutions to Support Clinical Integration and Care Coordination for Patients in the Rio Grande Valley

New Healthcare IT Platform Will Help Assure ACO Shared Savings Program Compliance and Success

DALLAS, TX -- (Marketwired) -- 07/23/14 -- Sandlot Solutions, a leading health interoperability and analytics provider, announced its partnership with Buena Vida y Salud, a newly-formed Accountable Care Organization (ACO) in South Texas. Buena Vida's 34 physicians serve a population of more than 150,000 patients in the Rio Grande Valley region.

Sandlot will construct a technology bridge to connect to Buena Vida's regional health information exchange (HIE), the Rio Grande Valley HIE. Sandlot's technology will allow Buena Vida to exchange data with hospitals and other healthcare organizations from across the region, facilitating care coordination and greater clinical integration for its entire patient population. The project, powered by Sandlot Connect, will go live within 90 days.

As a participant in the Center for Medicare & Medicaid Services' Shared Savings Program, Buena Vida will use Sandlot's platform to measure performance and compliance. The data collection and reporting Sandlot facilitates will enable Buena Vida to demonstrate it has met performance standards of the Shared Savings Program's 33 quality measures and qualify for any shared savings. Sandlot Dimensions and Sandlot Metrix will enable reporting against the metrics within four quality domains -- Patient/Caregiver Experience, Care Coordination/Patient Safety, Preventive Health and At-Risk Population -- to drive efficiency and transparency across Buena Vida's multiple physician practices. Additionally, Sandlot has experience working with other ACOs to automate the ACO Group Practice Reporting Option (GPRO) process, which collects data for approximately two-thirds of all the quality measures.
