Healthwise for Care Coordination Key Features
Healthwise for Care Coordination connects the entire care team, including the patient and caregivers, to evidence-based health education. This allows care teams to utilize consistent, relevant health content and helps patients understand and proactively manage their conditions. The app integrates with Salesforce Health Cloud, creating streamlined workflows to deliver the meaningful health content that drives patient success. Patients access their health education though the Salesforce Community Cloud, or via email, creating a cohesive patient experience and increasing the likelihood of care plan adherence. Every health education task is tracked on each patient's timeline so care coordinators can easily determine the next step in the care plan, while population-wide reports give organizations helpful insights into the overall impact of health education on patient outcomes.
