Replicor divulgue sa réussite à contrôler fonctionnellement HBV et HDV avec des NAP, ainsi que de nouveaux enseignements en pharmacologie NAP

Replicor Inc., une société biopharmaceutique à capitaux privés qui entend trouver un remède pour les infections chroniques au HBV et HDV, a divulgué aujourd'hui d'importantes nouvelles données sur l'activité des NAP dans les infections au HBV et HDV lors de la réunion annuelle de l'Association européenne pour l'étude du foie (EASL) 2017, qui se tient du 19 au 23 avril 2017 à Amsterdam (Pays-Bas).

Des mises à jour des études REP 301 (coinfection au HBV/HDV) et REP 401 (infection au HBV) ont démontré d'importantes avancées :

1. Dans l'étude REP 301, il a été démontré que le contrôle fonctionnel signalé précédemment lors du suivi à 24 semaines du HBV (5 patients sur 12) et du HDV (7 patients sur 12) a été maintenu à un an, démontrant ainsi la durabilité du contrôle fonctionnel établi avec la thérapie à base de NAP (affiche LBP-507).

2. Dans l'étude REP 401, 30 patients ont reçu au moins 12 semaines d'exposition à NAP, et la réponse au sérum HBsAg continue de s'améliorer au fil du temps : les réductions actuelles par rapport à la référence de base présentée étaient >1 log chez 29 patients, >2 logs chez 25 patients, >4 logs chez 19 patients, avec une perte de HBsAg chez 14 patients. Une efficacité améliorée de pegIFN (production des anti-HB marqués et/ou des poussées de transaminase thérapeutique) s'est produite chez tous les patients présentant une réduction de HBsAg > 4 log (affiche TH-154).

3. Étonnamment, l'analyse des sérums HBsAg, HBeAg, HBcrAg, HBV RNA, HBV DNA et HDV RNA dans les deux études indiquait que le HBsAg en circulation paraissait être presqu'entièrement dérivé de l'intégration.

« La notion selon laquelle presque tout le HBsAg en circulation est dérivé de l'intégration revêt d'importantes implications thérapeutiques », a dit le Dr. Andrew Vaillant, du CSO. « En tant que principal agent immunosuppressif des infections au HBV et HDV, la suppression de HBsAg sera essentielle. Il est très probable que de nouveaux agents expérimentaux possédant d'autres mécanismes antiviraux nécessiteront quand même des agents ciblant directement l'émission de HBsAg comme les NAP afin d'arriver à un taux élevé de contrôle fonctionnel. » Le Dr. Bazinet, PDG, d'ajouter: « nos données cliniques en expansion constante continuent de démontrer qu'il est possible d'atteindre des taux de contrôle fonctionnel importants du HBV et du HDV avec les médicaments et NAP actuellement approuvés. »

Un séquençage profond de HBsAg dans l'étude REP 102 n'a montré aucun pression de sélection avec REP 2139, validant ainsi à nouveau la réponse au sérum HBsAg à la thérapie NAP (afiche THU-155). Un système de culture de tissu récemment développé reproduit désormais pour la première fois les effets post-entrée des NAP sur la sécrétion des particules HBV observée précliniquement et cliniquement (affiche THU-156).

Le Dr. Vaillant a remarqué que « La capacité à modéliser les effets post-entrée des NAP dans la culture de tissu est la culmination de plus de 6 ans de travail qui a impliqué différents collaborateurs. Avec cet outil enfin en place, les mécanismes hôtes ciblés par les NAP peuvent désormais être explorés. »
